Screening for social determinants of health (SDoH) is a critical first step in helping patients overcome challenges to optimal care. With a direct connection to health equity, this webinar series will explore effective SDoH screening tools and processes, as well as resources to support hospital efforts. You’ll learn about screening requirements from the Centers for Medicare & Medicaid Services (CMS) to ensure compliance while you break down barriers to care.  

Offered in collaboration with Cynosure Health and four other state hospital associations, this webinar series will highlight the role of SDoH screenings in improving quality and health outcomes, particularly for care transitions. You’ll also hear about successful screening implementation strategies at hospitals and health systems across the U.S.

Introduction to SDoH Screening and the Impact on Quality of Care and Health Outcomes

An effective SDoH screening process has value to both patients and providers. This webinar will show you how through the lens of care transitions. You’ll also get clarity on CMS’s new SDoH screening requirement and the tools and resources available to support implementation.

Objectives

Speakers

Christina Boyd, MPH, CPHQ
IHA Director, Quality, Safety and Health Policy
Boyd has over 14 years of experience in hospital associations, working to advance quality and health outcomes in Illinois hospitals. In her current role, Boyd is responsible for leading and coordinating quality and patient safety initiatives to support the implementation of improvement processes and improve member performance. She is the lead for various federal, state and local quality improvement projects, as well as the topic lead for patient and family engagement and EMS operations and policy development. 
 
Natalie Graves, MPH
Director, Research and Implementation, Cynosure Health
Graves has over a decade of experience in leading programs to transform healthcare by leveraging quality improvement strategies and promoting peer-to-peer learning. She has collaborated with the Centers for Medicare & Medicaid Services, hospitals, accountable care organizations, state Medicaid agencies, and patient and family advocates to translate ambitious goals into concrete action plans for improvement. Her areas of expertise include value-based care, delivery system transformation, payment reform, quality improvement, project management, and coaching and mentoring.
 
Jenny Winkler, MPH, CIC, CPPS
IHA Senior Director, Quality, Safety and Health Policy
With over 14 years of experience in healthcare, Winkler has extensive knowledge and expertise in the areas of infection prevention, patient safety, quality improvement and project management. Through IHA’s Institute in Innovations in Care and Quality, she works with hospitals to support and enhance their patient safety and quality work.
